How to Request Your Medical Records (Subject Access Request)
You have the right to access your personal medical records under the UK General Data Protection Regulation (UK GDPR). This is called a Subject Access Request (SAR). To request a copy of your records, please follow the steps below:

Step 1: Prepare Your Request
You can make your request in writing (letter or email). Please include the following information:

Your full name, date of birth, and current address

Any previous names or addresses (if relevant to the records)

A clear description of the information you are requesting (e.g. all records, or records from a specific time period or department)

How you would like to receive the records (e.g. printed copy, secure email)

Please let us know if you do not intend to raise a concern against Body Balance. If you do have a concern please read our policy on dealing with your concerns.​
Step 2: Provide Proof of Identity
To protect your privacy, we need to verify your identity. Please include a copy of:

One form of photo ID (e.g. passport or driving licence), and

One proof of address (e.g. recent utility bill or bank statement dated within the last 3 months)

Step 4: Response Time
We will respond to your request within one month of receiving it. In some cases, this may be extended by up to two months if the request is complex, but we will let you know if this applies.

SARs are usually free of charge. However, we may charge a reasonable fee if your request is excessive or repetitive.

You may request records on behalf of someone else (e.g. a child or someone you have legal authority for)

in this case, please also include proof of your authority to act on their behalf (e.g. parental responsibility, power of attorney).
